HypeTrain Digital and Ice-Pick Lodge have recently announced that Pathologic 3 will be released on Steam for PC on January 9, 2026 (19 December 1404).
This game is a new part of the popular series that revolves around a mysterious plague in a remote city in the endless plains, and is actually a psychological detective story where you play as a doctor named Daniil Dankovsky, who has come to the city to meet an immortal man, but is forced to enter into a 12-day battle to save the people from the epidemic. In fact, you have 12 days to save a city plagued by a mysterious plague. As a doctor, you have to examine and interrogate patients to distinguish lies from truth and decide who to save. Of course, things don’t always go according to plan, and discovering the secret of the plague is a priority.

The indie game Pathologic, released in 2005, is famous for its complex story and high difficulty. Despite the high difficulty, players who make it to the end are rewarded with an unforgettable experience. After the HD remastered version in 2015, the studio launched a project on the Kickstarter platform to make a new part called Pathologic 2.
The second part was actually not a direct sequel to the first game, but a complete recreation of it. One of its notable changes was that it only followed the story of one of the three playable characters of the original episode. While the second game focused on Haruspex’s journey, the other two characters, the Bachelor and Changeling, only made brief appearances, with the promise that their full stories would be told in the future. Apparently, in the new episode, the bachelor, Daniil Dankovsky, will be the main character of the game.
Finally, it should be mentioned that the PlayStation 5 and Xbox Series X/S versions of the third game are also under development, and the new game demo is now available at the Steam Next Fest. You can see the published trailer below.
